Send BWC Your W-9

As a requirement of the Internal Revenue Service (IRS), we need a current W-9 tax form on file for your business. Please include your BWC-issued policy number in Line 7, which asks for your account number. This allows us to match your W-9 to your business.

You can upload the form on our website. Go to the Upload W-9 service offering where you'll be asked to sign in.

Mail
If you’d rather download and fill out the form, visit the IRS website for a fillable PDF version. Then print it out and mail it to:
